 Indication & Scope

FuLaiDi®/NovoNorm® is a prescription meglitinide antidiabetic agent indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is suitable for patients whose blood glucose remains inadequately controlled despite lifestyle modification and may be used alone or in combination with metformin. Repaglinide provides rapid, short-acting stimulation of insulin secretion and is particularly effective for controlling postprandial blood glucose excursions.

  • Mechanism: Repaglinide stimulates insulin release by binding to ATP-sensitive potassium channels on pancreatic β-cells, causing membrane depolarization, calcium influx, and rapid insulin secretion.
  • Drug class: Meglitinide (non-sulfonylurea insulin secretagogue).
  • Clinical use: Treatment of adults with type 2 diabetes mellitus as monotherapy or in combination with metformin when glycemic control is inadequate. Not indicated for type 1 diabetes mellitus or diabetic ketoacidosis.

How to use ?

Recommended Dose: The usual starting dose is 0.5 mg before each main meal. The dose should be individualized according to blood glucose response. The maximum single dose is 4 mg, and the maximum total daily dose is 16 mg.

 

Time: Take within 30 minutes before meals, preferably immediately before eating.

 

Instructions for use: Swallow the tablet with water before each main meal. Skip the dose if a meal is skipped to reduce the risk of hypoglycemia. Continue diet and exercise throughout treatment, and monitor blood glucose regularly to adjust the dose when necessary.

 

Interaction: The glucose-lowering effect may be enhanced by metformin, insulin, DPP-4 inhibitors, SGLT2 inhibitors, salicylates, and β-blockers, while corticosteroids, thiazide diuretics, thyroid hormones, and oral contraceptives may reduce its efficacy. Concomitant use with gemfibrozil is contraindicated because it markedly increases repaglinide exposure and the risk of severe hypoglycemia.

 

Side effects: Common adverse reactions include hypoglycemia, abdominal pain, diarrhea, nausea, constipation, headache, dizziness, and upper respiratory tract infection. Less common reactions include elevated liver enzymes, visual disturbances, hypersensitivity reactions, and skin rash.
