 Indication & Scope

Vericiguat is for adult patients with symptomatic chronic heart failure with reduced ejection fraction who have experienced a recent heart failure exacerbation, belonging to the therapeutic field of cardiovascular diseases, to lower risks of cardiovascular death and heart failure hospitalization.

  • Drug class
    Oral soluble guanylate cyclase (sGC) stimulator, novel cardiovascular anti-heart failure agent.
  • Mechanism
    It directly stimulates soluble guanylate cyclase independent of nitric oxide, elevates intracellular cGMP levels to dilate blood vessels, reduce cardiac preload and afterload, relieve myocardial wall stress and inhibit ventricular remodeling, improving cardiac function without relying on endogenous NO pathway.
  • Clinical use
    Once daily oral dose starting at 2.5 mg, titrated to 10 mg per tolerance. The VICTORIA Phase III trial proved it cuts cardiovascular death/HF hospitalization risks for decompensated HFrEF patients with steady long-term safety, used as add-on to standard HF treatments.

How to use ?

   Recommended Dose:  Start with 2.5 mg once daily. After 2 weeks, increase to 5 mg once daily if tolerated; further titrate to the target dose of 10 mg once daily after another 2 weeks based on blood pressure and tolerance. Do not exceed 10 mg daily.

   Time:Take orally at the same time every day, with or without food.

   Instructions for use: Swallow whole tablet with water; do not split, crush or chew. Take missed dose promptly, no double dose. Stop use and see doctor for lasting symptomatic hypotension.

   Interaction: Strong PDE5 inhibitors are forbidden due to severe hypotension risk. Combined antihypertensives may lower blood pressure excessively and require close monitoring; no notable CYP450 interactions exist.

   Side effects: Common side effects have Hypotension, anemia, dizziness.Rare serious adverse reactions have Syncope, orthostatic hypotension, severe symptomatic low blood pressure, gastrointestinal discomfort.
