Indication & Scope
MYQORZO® (Aficamten) is indicated for the treatment of adult patients with symptomatic obstructive hypertrophic cardiomyopathy (HCM), NYHA Class II–III, to improve exercise capacity and clinical symptoms.
- Mechanism: Aficamten is a reversible cardiac myosin inhibitor that reduces excessive myocardial contractility by inhibiting actin–myosin cross-bridge formation, thereby decreasing left ventricular outflow tract (LVOT) obstruction.
- Drug Class: Cardiac myosin inhibitor.
- Clinical Use: Used for adults with symptomatic obstructive HCM to improve functional capacity and disease-related symptoms.

How to use ?
Recommended Dose: The recommended starting dose of MYQORZO is 5 mg orally once daily. The dose may be increased in 5 mg increments every two weeks based on echocardiographic assessment of cardiac function. The maximum recommended dose is 20 mg once daily.
Time: Administer orally once daily. Swallow the tablet whole with water.
Instructions for use: Regular echocardiographic monitoring is required throughout treatment to evaluate left ventricular ejection fraction (LVEF) and left ventricular outflow tract gradient (LVOT-G). Treatment should not be initiated in patients with LVEF below 55%. If clinically significant reductions in LVEF occur during therapy, treatment interruption or dose adjustment should be considered according to the approved prescribing information.
Interaction: Strong CYP3A4 inhibitors may increase systemic exposure to Aficamten and require dose adjustment, while strong CYP3A4 inducers may reduce drug exposure and decrease therapeutic efficacy. Patients should inform their healthcare provider of all concomitant medications before starting treatment.
Side effects: Common adverse reactions include dizziness, fatigue, palpitations, nausea, headache, and peripheral edema. Important safety risks include reduced left ventricular ejection fraction (LVEF) and heart failure, making regular cardiac monitoring essential throughout treatment.
