Indication & Scope

ZOCOR® is a prescription HMG-CoA reductase inhibitor (statin) indicated as an adjunct to diet for reducing elevated total cholesterol, LDL cholesterol, triglycerides, and apolipoprotein B in patients with primary hypercholesterolemia, heterozygous familial hypercholesterolemia, mixed dyslipidemia, and homozygous familial hypercholesterolemia. It is also indicated for reducing the risk of major cardiovascular events in patients with coronary heart disease or those at high cardiovascular risk.

Mechanism: Simvastatin inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis, resulting in increased LDL receptor expression and enhanced clearance of circulating LDL cholesterol.
Drug class: HMG-CoA reductase inhibitor (Statin).
Clinical use: Treatment of hypercholesterolemia and mixed dyslipidemia, and prevention of myocardial infarction, stroke, coronary revascularization, and progression of atherosclerotic cardiovascular disease.

How to use ?

Recommended Dose: For hypercholesterolemia, the usual starting dose is 10–20 mg once daily in the evening. Patients requiring greater LDL-C reduction may start at 20–40 mg daily. The maximum recommended dose is 40 mg once daily.

Time: Take once daily in the evening, with or without food.

Instructions for use: Swallow the tablet whole or split if needed. Continue a cholesterol-lowering diet during treatment. Dose adjustments should be based on lipid response.

Interaction: Avoid strong CYP3A4 inhibitors, gemfibrozil, cyclosporine, danazol, large amounts of grapefruit juice, and other medicines that increase the risk of myopathy or rhabdomyolysis.

Side effects: Common adverse reactions include myalgia, abdominal pain, constipation, nausea, headache, elevated liver enzymes, and muscle weakness. Rare but serious reactions include myopathy and rhabdomyolysis.
