Humalog Mix50® is a biphasic insulin analog containing 50% rapid-acting insulin lispro and 50% intermediate-acting insulin lispro protamine suspension, indicated for improving glycemic control in adults with type 1 or type 2 diabetes mellitus requiring insulin therapy. The rapid-acting component controls postprandial blood glucose excursions, while the protamine suspension provides prolonged basal insulin coverage throughout the day. It is particularly suitable for patients with prominent post-meal hyperglycemia.
- Mechanism: Insulin lispro rapidly promotes peripheral glucose uptake and suppresses hepatic glucose production after meals, while insulin lispro protamine suspension slowly releases insulin to provide intermediate-duration basal glycemic control.
- Drug class: Biphasic insulin analog (premixed insulin analogue).
- Clinical use: Glycemic control in adults with type 1 or type 2 diabetes mellitus requiring insulin therapy. May be used alone or together with oral antidiabetic agents such as metformin when clinically appropriate.

How to use ?
Recommended Dose: Individualized according to blood glucose monitoring and clinical response. Humalog Mix50® is typically administered once or twice daily.
Time: Inject within 15 minutes before a meal.
Instructions for use: For subcutaneous injection only. Inject into the abdomen, thigh, upper arm, or buttock, rotating injection sites regularly to reduce the risk of lipodystrophy. Gently roll and invert the cartridge until the suspension appears uniformly milky before each injection. Do not administer intravenously or via an insulin infusion pump.
Interaction: The glucose-lowering effect may be enhanced by other antidiabetic medications, including metformin, sulfonylureas, GLP-1 receptor agonists, and SGLT2 inhibitors, while corticosteroids, thyroid hormones, sympathomimetics, and certain diuretics may reduce its efficacy. Dose adjustment and regular blood glucose monitoring may be required.
Side effects: The most common adverse reactions include hypoglycemia, injection-site reactions, lipodystrophy, weight gain, edema, and, rarely, allergic reactions.
