 Indication & Scope

XinKeQing® Higenamine Hydrochloride Injection is used as a pharmacological stress agent for cardiac function stress imaging examinations, to induce controlled cardiac load and assist diagnosis of myocardial ischemia lesions during nuclear medicine or ultrasound cardiac scans.

  • Mechanism: Higenamine hydrochloride is a selective beta-adrenergic receptor partial agonist. Continuous intravenous infusion activates cardiac β1 receptors to accelerate heart rate, raise myocardial contractility and increase myocardial oxygen demand, simulating physical exercise cardiac load without patient movement. This induced myocardial oxygen supply-demand imbalance can expose hidden ischemic myocardial lesions visible on cardiac imaging equipment.
  • Drug class: Cardiac stress diagnostic agent; selective β-adrenoceptor partial agonist injectable.
  • Clinical use: Alternative stress substitute for patients unable to complete treadmill exercise cardiac stress tests. How to use ?

Recommended Dose: Graded continuous intravenous infusion for 3 minutes at escalating flow rates: 0.5, 1.0, 2.0 and 4.0 μg per kilogram of patient body weight per minute, under real-time ECG monitoring throughout infusion.

 

Time: Start intravenous infusion of the injection 10 minutes before formal cardiac imaging scanning, maintain graded infusion during the entire image acquisition process.

 

Instructions for use: Only medical staff with cardiac emergency training can perform infusion and monitoring. Equip emergency anti-arrhythmia and resuscitation drugs on standby during all injection procedures. Monitor blood pressure, heart rate and real-time ECG continuously throughout infusion. Immediately stop infusion if severe palpitations, chest pain or hypotension occurs. Discontinue all beta-blocker heart medicines at least 48 hours before scheduled stress imaging.

 

Interaction: Concomitant beta-blockers fully neutralize higenamine’s cardiac load-inducing effect and must be paused before examination. Combined anticoagulant medicines slightly raise minor bleeding risk at the intravenous injection puncture site. Anti-hypertensive calcium channel blockers may amplify hypotension side effects during infusion.

 

Side effects:

1. Common mild transient adverse reactions: palpitations, mild flushing, slight headache, temporary tachycardia, mild chest tightness that resolves quickly after stopping infusion.

2. Severe rare adverse reactions: sustained severe arrhythmia, sharp hypotension, severe angina pectoris attack, acute myocardial ischemia exacerbation.
