Indication & Scope
PingYiDa®/JANUMET® is a prescription combination antidiabetic medicine indicated, as an adjunct to diet and exercise, to improve glycemic control in adults with type 2 diabetes mellitus when metformin alone does not provide adequate glycemic control or in patients already receiving sitagliptin and metformin as separate tablets. The combination provides complementary mechanisms of action by improving glucose-dependent insulin secretion while reducing hepatic glucose production and improving insulin sensitivity.
- Mechanism: Sitagliptin is a selective DPP-4 inhibitor that increases endogenous incretin hormones (GLP-1 and GIP), enhancing glucose-dependent insulin secretion and suppressing glucagon release. Metformin decreases hepatic glucose production, reduces intestinal glucose absorption, and improves peripheral insulin sensitivity.
- Drug class: DPP-4 inhibitor / Biguanide combination.
- Clinical use: Treatment of adults with type 2 diabetes mellitus inadequately controlled with metformin alone or already treated with sitagliptin and metformin combination therapy. Not indicated for type 1 diabetes or diabetic ketoacidosis.

How to use ?
Recommended Dose: The dosage should be individualized according to the patient’s current antidiabetic regimen, glycemic control, and renal function. The maximum recommended daily dose is sitagliptin 100 mg and metformin hydrochloride 2,000 mg.
Time: Take orally twice daily with meals to reduce gastrointestinal side effects.
Instructions for use: Swallow the tablets whole with water during meals. Do not crush, chew, or split the tablets. Continue diet and exercise during treatment. Renal function should be assessed before treatment and monitored periodically, especially in elderly patients.
Interaction: The risk of hypoglycemia may increase when used with insulin or sulfonylureas. Alcohol, iodinated contrast media, and nephrotoxic drugs may increase the risk of metformin-associated lactic acidosis. Corticosteroids, diuretics, and thyroid hormones may reduce glucose-lowering efficacy.
Side effects: Common adverse reactions include diarrhea, nausea, abdominal discomfort, decreased appetite, headache, nasopharyngitis, upper respiratory tract infection, and hypoglycemia when used with insulin or sulfonylureas. Rare but serious adverse reactions include lactic acidosis, acute pancreatitis, severe hypersensitivity reactions, and bullous pemphigoid.
