Indication & Scope

KeLi® is a prescription cardiac glycoside indicated for the treatment of heart failure and for ventricular rate control in patients with atrial fibrillation or atrial flutter. It may also be used in selected patients with paroxysmal supraventricular tachycardia (PSVT). Because digoxin has a narrow therapeutic index, careful dose adjustment and therapeutic drug monitoring are recommended.

Mechanism: Digoxin inhibits the sodium-potassium ATPase pump, increasing intracellular calcium in cardiac myocytes to enhance myocardial contractility. It also increases vagal tone, slowing atrioventricular (AV) nodal conduction and reducing ventricular rate.
Drug class: Cardiac glycoside.
Clinical use: Treatment of chronic heart failure with reduced systolic function and ventricular rate control in atrial fibrillation or atrial flutter.

How to use ?

Recommended Dose: The usual maintenance dose for adults is 0.125–0.25 mg once daily. Lower doses are recommended for elderly patients and those with renal impairment. Loading doses should only be used when clinically indicated.

Time: Take once daily, preferably at the same time each day, with or without food.

Instructions for use: Swallow the tablet with water. Regularly monitor serum digoxin concentration, renal function, electrolytes (especially potassium and magnesium), and ECG during treatment.

Interaction: Drugs such as amiodarone, verapamil, quinidine, macrolide antibiotics, and spironolactone may increase digoxin concentrations. Loop and thiazide diuretics can increase the risk of digoxin toxicity by causing hypokalemia.

Side effects: Common adverse reactions include nausea, vomiting, loss of appetite, dizziness, fatigue, and visual disturbances (such as blurred or yellow vision). Serious adverse effects include bradycardia, AV block, ventricular arrhythmias, and digoxin toxicity.
