 Indication & Scope

Jascayd® Nerandomilast Tablets treats adult patients diagnosed with idiopathic pulmonary fibrosis (IPF) to slow progressive fibrotic lung tissue damage and decline in pulmonary function.

  • Mechanism: Nerandomilast is a selective PDE4B phosphodiesterase inhibitor. It elevates intracellular cAMP concentrations in lung fibroblasts and inflammatory immune cells, suppresses pro-fibrotic cytokine release, inhibits fibroblast proliferation and extracellular matrix collagen deposition, delivering combined anti-inflammatory and anti-fibrotic pulmonary protective effects.
  • Drug class: Selective PDE4B inhibitor; oral anti-fibrotic respiratory therapeutic agent.
  • Clinical use: Long-term maintenance treatment for idiopathic pulmonary fibrosis; slows forced vital capacity decline, reduces acute exacerbation risk and improves patient exercise tolerance.

How to use ?

Recommended Dose: 18 mg oral twice daily, approximately 12 hours apart. Reduce to 9 mg twice daily for persistent gastrointestinal adverse reactions.

 

Time: Administer morning and evening at consistent 12-hour intervals, with or without meals.

 

Instructions for use: Tablets may be swallowed whole or dispersed in room-temperature water for patients with swallowing difficulty. Baseline liver function testing is required before therapy initiation.

 

Interaction: Systemic CYP3A4 inhibitors increase nerandomilast plasma levels and gastrointestinal side effect frequency; concurrent other PDE4 inhibitors are contraindicated.

 

Side effects:

1. Common adverse reactions: diarrhea, nausea, abdominal pain, headache, weight loss.

2. Severe rare side effects: severe psychiatric mood disorders, significant hepatic enzyme elevation.
