Indication & Scope
ActIns® is a prescription thiazolidinedione (TZD) insulin sensitizer indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It may be used as monotherapy or in combination with metformin, sulfonylureas, or insulin when additional glycemic control is required. It is not indicated for the treatment of type 1 diabetes mellitus or diabetic ketoacidosis.
- Mechanism: Pioglitazone is a selective peroxisome proliferator-activated receptor gamma (PPAR-γ) agonist that improves insulin sensitivity in adipose tissue, skeletal muscle, and the liver, thereby reducing insulin resistance and hepatic glucose production.
- Drug Class: Thiazolidinedione (TZD) insulin sensitizer.
- Clinical Use: Improvement of glycemic control in adults with type 2 diabetes mellitus as monotherapy or in combination with other antidiabetic agents.

How to use ?
Recommended Dose: The recommended starting dose is 15–30 mg once daily. The dose may be increased to a maximum of 45 mg once daily according to glycemic response.
Time: Take once daily, with or without food.
Instructions for use: Swallow the tablet whole with water. It may be used alone or in combination with other antidiabetic medications. Regular monitoring of blood glucose and liver function is recommended.
Interaction: The glucose-lowering effect may be enhanced when combined with insulin or sulfonylureas, increasing the risk of hypoglycemia. CYP2C8 inhibitors (e.g., gemfibrozil) may increase pioglitazone exposure, while CYP2C8 inducers (e.g., rifampin) may reduce its effectiveness.
Side effects: Common adverse reactions include edema, weight gain, headache, upper respiratory tract infection, and anemia. When used with insulin or sulfonylureas, hypoglycemia may occur. Serious adverse reactions include heart failure, hepatic dysfunction, macular edema, and increased fracture risk.
