Ivonescimab Injection is a first-line treatment for locally advanced or metastatic non-squamous non-small cell lung cancer.

  • Mechanism: Bispecific antibody targeting PD-1 and VEGF. It blocks PD-1 immune checkpoint to activate anti-tumor T-cell immunity, while inhibiting VEGF-mediated tumor angiogenesis to suppress tumor growth and metastasis.
  • Drug class: Bispecific monoclonal antibody, immunotherapy anti-neoplastic agent.
  • Clinical use: First-line monotherapy or combination chemotherapy for unresectable locally advanced/metastatic non-squamous NSCLC without actionable driver gene mutations.

How to use ?

Recommended Dose: 1800 mg intravenously once every 3 weeks. Dose delay or permanent discontinuation for grade 3+ immune-related adverse events.

 

Time: Intravenous infusion over 120 minutes on day 1 of each 21-day treatment cycle.

 

Instructions for use:

1. Premedication is not routinely required for first infusion.

2. Monitor patients closely for immune-related adverse events throughout treatment.

3. Hold treatment for severe immune toxicities and initiate corticosteroid rescue therapy if needed.

4. Do not mix with other infusion medications in the same IV line.

 

Interaction:

1. Systemic corticosteroids and immunosuppressants reduce anti-tumor efficacy.

2. Concurrent chemotherapy increases the risk of myelosuppression.

3. No direct CYP450 metabolic interactions with small-molecule drugs.

 

Side effects:

1. Common: Hypertension, fatigue, hypothyroidism, diarrhea, rash, infusion-related reactions.
2. Severe: Immune-related pneumonitis, colitis, hepatitis, myocarditis, severe hemorrhage from anti-VEGF activity.
