 Indication & Scope

Glurenorm® is a prescription second-generation sulfonylurea indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It stimulates pancreatic β-cells to increase insulin secretion and is particularly suitable for patients with mild renal impairment because the drug is predominantly eliminated via the biliary tract.

  • Mechanism: Gliquidone stimulates insulin release by binding to sulfonylurea receptors (SUR1) on pancreatic β-cells, leading to closure of ATP-sensitive potassium channels, membrane depolarization, calcium influx, and insulin secretion.
  • Drug class: Second-generation sulfonylurea.
  • Clinical use: Treatment of adults with type 2 diabetes mellitus inadequately controlled by diet and exercise alone, especially patients with mild renal impairment. Not indicated for type 1 diabetes, diabetic ketoacidosis, or diabetic coma.

How to use ?

Recommended Dose: The usual starting dose is 15–30 mg daily before breakfast. The dosage should be individualized according to blood glucose control. Daily doses above 30 mg should be divided before meals. The maximum recommended daily dose is 180 mg.

 

Time: Take approximately 30 minutes before meals.

 

Instructions for use: Swallow the tablet with water before meals. The tablet may be divided if necessary. Continue dietary management and regular exercise throughout treatment. Regular blood glucose monitoring is recommended, particularly in elderly patients and those at risk of hypoglycemia.

 

Interaction: The glucose-lowering effect may be enhanced by insulin, metformin, DPP-4 inhibitors, SGLT2 inhibitors, salicylates, and β-blockers, while corticosteroids, thiazide diuretics, oral contraceptives, and thyroid hormones may reduce its efficacy. β-blockers may also mask symptoms of hypoglycemia.

 

Side effects: Common adverse reactions include hypoglycemia, dizziness, sweating, tremor, gastrointestinal discomfort, nausea, abdominal fullness, skin rash, pruritus, and headache. Rare adverse reactions include hepatic dysfunction, thrombocytopenia, leukopenia, and allergic reactions.
