 Indication & Scope

Caspofungin Acetate irreversibly inhibits fungal β-(1,3)-D-glucan synthase to block synthesis of fungal cell wall component glucan, causing fungal cell rupture and death. Human cells lack this target enzyme, so the drug has minimal renal toxicity compared with polyene antifungals.

How to use ?

Recommended Dose: Intravenous slow infusion only, 60-minute infusion duration; intravenous bolus injection is forbidden.

 

Time: Loading dose: 70mg once on Day 1; Maintenance dose: 50mg once daily afterwards; No loading dose required for esophageal candidiasis.

 

Instructions for use: Adjust daily maintenance dose to 70mg when combined with CYP450 inducers. Monitor liver function, serum creatinine and serum electrolytes regularly throughout treatment.

 

Interaction: CYP450 enzyme inducers (rifampicin, dexamethasone, phenytoin, carbamazepine) accelerate drug clearance, maintenance dose shall be adjusted to 70mg daily; compatible only with normal saline, dextrose-containing diluents are prohibited; do not mix with other intravenous medicines.

 

Side effects: Fever, headache, nausea, diarrhea, elevated liver transaminase, hypokalemia, rash.
