Description
Indication & Scope
ShiYao/DuoMiFen® Peramivir Injection provides intravenous antiviral treatment for acute uncomplicated seasonal influenza A and influenza B virus infections in adult and pediatric patients aged 6 months and above, with symptom onset within 48 hours. It is also applicable for severe influenza infection and influenza patients unable to take oral antiviral medications due to vomiting or poor gastrointestinal tolerance.
- Mechanism: Peramivir is a selective, potent competitive influenza virus neuraminidase inhibitor. It specifically binds to the active site of influenza A and B neuraminidase, blocking enzyme activity required for viral budding, proliferation and diffusion from infected host cells. It effectively inhibits new influenza virus particle release and prevents further viral transmission and in vivo replication, rapidly controlling influenza progression.
- Drug class: Systemic anti-influenza viral agent; selective neuraminidase inhibitor; intravenous broad-spectrum anti-influenza small-molecule drug.
- Clinical use: First-line rapid intravenous treatment for acute influenza infection. It achieves fast viral load reduction and rapid relief of influenza fever, myalgia, cough and fatigue symptoms. Suitable for severe influenza cases, inpatient influenza treatment, and patients with oral drug intolerance or poor oral absorption, providing more stable antiviral efficacy than oral anti-influenza agents.

How to use ?
Recommended Dose: Dose adjustment is required for patients with renal insufficiency according to creatinine clearance rate.
Time: Initiate intravenous infusion treatment as early as possible within 48 hours after the appearance of influenza fever and respiratory symptoms to ensure optimal antiviral efficacy. No meal time restriction for administration.
Instructions for use: Dilute the concentrated peramivir solution with normal saline before intravenous infusion, complete infusion within 15 to 30 minutes. Strictly adjust dosage for patients with impaired renal function. Closely monitor vital signs during infusion to prevent allergic reactions. Observe pediatric and adolescent patients for abnormal neurobehavioral changes after administration.
Interaction: No obvious pharmacokinetic antagonism or synergism with common antibiotics, antipyretic drugs and respiratory symptomatic treatment medications. Concurrent renal toxic drugs may increase renal excretion burden and require renal function monitoring. No significant drug interaction with other influenza antiviral agents.
Side effects:
1. Common mild adverse reactions: transient diarrhea, vomiting, mild nausea, headache, dizziness and slight neutropenia, which are self-limiting and reversible.
2. Severe rare adverse reactions: severe anaphylactic shock, severe skin allergic reactions, acute renal function injury, abnormal neuropsychiatric behaviors in minor patients, severe gastrointestinal bleeding.

FAQ
What core advantage makes intravenous Peramivir Injection superior to common oral anti-influenza drugs for severe influenza patients?
Peramivir Injection achieves rapid and complete systemic antiviral effect via intravenous infusion without relying on gastrointestinal absorption, suitable for severe influenza patients and those with vomiting and oral drug intolerance.
What is the optimal treatment time window for starting Peramivir Injection infusion after influenza symptom onset?
Treatment should be initiated within 48 hours after fever and respiratory symptom occurrence to maximize viral inhibition efficacy and shorten influenza disease course.
Which patient groups need personalized dose reduction when receiving Peramivir Injection anti-influenza infusion?
Patients with renal insufficiency and impaired creatinine clearance require mandatory dose adjustment to avoid drug accumulation and adverse toxic reactions.
What main mild discomforts may occur after Peramivir Injection influenza infusion and how do they resolve?
Mild transient gastrointestinal symptoms like diarrhea and nausea may appear, which can recover spontaneously after drug metabolism without additional targeted treatment in most cases.
