Indication & Scope
Treatment of mild cognitive impairment and mild dementia caused by Alzheimer’s disease with confirmed amyloid-beta pathology.
- Mechanism: Monoclonal antibody targeting soluble aggregated amyloid-beta protofibrils in the brain, clearing amyloid plaques and slowing neurodegeneration in early AD.
- Drug class: Anti-amyloid beta monoclonal antibody; disease-modifying anti-Alzheimer agent.
- Clinical use: Slow cognitive and functional decline in early-stage Alzheimer’s patients with verified brain amyloid deposition.

How to use ?
Recommended Dose: 10 mg/kg intravenous infusion every 2 weeks; subcutaneous 500 mg weekly for initiation, 360 mg weekly maintenance.
Time: Infusion takes 1 hour; fixed biweekly/weekly schedule, consistent timing recommended.
Instructions for use:
1. Confirm amyloid pathology test before treatment.
2. Dilute vial with 0.9% NaCl for IV infusion; use 0.2μm inline filter.
3. Monitor brain MRI regularly to screen ARIA.
4. Do not administer rapid bolus injection.
Interaction:
1. Anticoagulants (warfarin, apixaban) raise intracranial bleeding risk.
2. Antiplatelet drugs increase ARIA hemorrhage probability.
3. No direct metabolic CYP450 interaction.
Side effects:
1. Common: Headache, infusion reactions, ARIA (brain edema/bleeding), fall, diarrhea.
2. Severe: Severe intracranial hemorrhage, anaphylaxis, seizures, worsening cognitive function from ARIA.

What is the controvercy with Lecanemab?
One of the major safety concerns with Leqembi is the risk of ARIA, which includes brain swelling (ARIA‑E) and small or larger brain bleeds (ARIA‑H).
Does Lecanemab really work?
Leqembi is not a cure for Alzheimer’s disease. Nor is it approved for all patients. More on that later. But in a phase 3 clinical trial of some 1,900 patients, the drug demonstrated success in slowing cognitive and functional decline, compared to a placebo.
How long do you take Lecanemab?
After 18 months of treatment, talk to your healthcare provider about how you are doing on Lecanemab and when might be the right time to reevaluate your treatment schedule.
Who should not take Lecanemab?
Taking it alongside blood-thinning or clot-busting medications can increase the risk of severe brain bleeds. Having a pre-existing brain condition or two copies of the ApoE ε4 gene can also increase the risk of serious brain swelling and bleeding.
