Indication & Scope
VANCOCIN® is a prescription glycopeptide antibiotic indicated for the treatment of serious infections caused by susceptible Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA), enterococci, and other susceptible organisms. It is used for septicemia, infective endocarditis, osteomyelitis, severe skin and soft tissue infections, lower respiratory tract infections, and other severe Gram-positive infections when β-lactam antibiotics are ineffective or contraindicated. Oral vancomycin is indicated for the treatment of Clostridioides difficile-associated diarrhea and staphylococcal enterocolitis.
- Mechanism: Vancomycin inhibits bacterial cell wall synthesis by binding with high affinity to the D-alanyl-D-alanine terminus of cell wall precursor units, resulting in bacterial cell death.

How to use ?
Recommended Dose: The usual adult dose is 500 mg every 6 hours or 1 g every 12 hours by intravenous infusion. Pediatric dosing is generally 40 mg/kg/day divided into 2–4 doses. Oral vancomycin is indicated only for Clostridioides difficile-associated diarrhea. Dosage should be adjusted according to renal function and therapeutic drug monitoring.
Time: Administer by intravenous infusion over at least 60 minutes.
Instructions for use: Reconstitute and dilute before intravenous infusion. Do not administer by rapid IV injection or intramuscular injection. Monitor renal function and vancomycin serum concentrations during prolonged therapy.
Interaction: Concomitant use with aminoglycosides, amphotericin B, cisplatin, or other nephrotoxic or ototoxic agents may increase toxicity. Cholestyramine may reduce the efficacy of oral vancomycin.
Side effects: Common adverse reactions include Red Man Syndrome, phlebitis, rash, nausea, and fever. Serious adverse reactions include nephrotoxicity, ototoxicity, severe hypersensitivity reactions, and severe skin reactions.

VANCOCIN® (Vancomycin Hydrochloride for Injection) is primarily used for serious Gram-positive bacterial infections, especially MRSA, enterococcal infections, infective endocarditis, osteomyelitis, septicemia, and severe pneumonia.
Why must VANCOCIN® (Vancomycin Hydrochloride for Injection) be infused slowly?
Rapid infusion may cause Red Man Syndrome, characterized by flushing, itching, hypotension, and rash. Infusion over at least 60 minutes significantly reduces this risk.
Can intravenous VANCOCIN® (Vancomycin Hydrochloride for Injection) treat Clostridioides difficile infection?
No. Intravenous vancomycin does not achieve effective intestinal concentrations. Oral vancomycin should be used for C. difficile-associated diarrhea.
Is therapeutic drug monitoring necessary?
Yes. Monitoring vancomycin serum concentrations is recommended, particularly in patients with severe infections, renal impairment, prolonged treatment, or concomitant nephrotoxic medications.
